Welcome, history is short to understand, you need to tell the age of the patient, what other medical history is there? like diabetes, hypertension, cholesterol levels. and blood reports like CBC, Liver function tests, urea, creatinine, electrolytes, his MRI brain, what medication patient is taking, and his health status, due to medical conditions and/or neurological condition patient secondarily develops neuropsychiatric symptoms like hallucinations, delusions, and these symptoms fluctuates from time to time and one more condition occurs in this condition which is called Delirium( acute confusional state), you need to be assessed properly to understand these symptoms. Thank you
