Good day Doctor, Let me explain my situation I been collapsed on the floor in last month two time. (Feel dizziness) and I’m suffering with a pain on my chest and same way back middle of the shoulder and I feel very bitter taste specially when I have meal . I have done all the test below Chest X-ray: Normal Electrocardiography (twice): Normal Ultrasound: Mild fatty liver Endoscopy (biopsy sample was taken during the test) Mild gastritis Cholesterol: Cholesterol 190. HDL 42. LDL132. Triglycerides 155. Blood sugar level: 93 mg/dl Stool test: Normal Uric acid: 6.9 mg/dl Also many more blood test recommended by gastrologist doctor. I'm eating extremely high dose medicines from more than 2 months. But my condition is still quite same. Mouth taste bitter and pain on the back (left side nearby shoulder). Could you please advise. Thanks in Advance.

You need Holter monitoring to document any dysrythmia.ECG shows changes only during the attack can be otherwise normal.This bitter taste in mouth could be reflux related.Please visit with your labs and prescription for proper evaluation.

First up, we need to be absolutely sure that you do not have cardiac issues. A clear ECG doesn't mean anything even when performed during a pain episode. It should be accompanied by a blood test for your cardiac enzymes: Troponin I or T if you're getting them done immediately and CK-MB if its been a few hours and the entire cardiac enzymes if its been longer. Even if all that is clear an echocardiogram and an Exercise Tolerance Test, in which they perform you ECG while you exercise on a treadmill, would be better to rule out cardiac problem. I do not think you have a heart issue but I would never bet my money on it and would always want to be certain that such is the case.
Now knowing that you are a single male in Dubai, I am going to assume certain things, so you will have to correct me if I am wrong.
You are alone in Dubai with family in Pakistan. You are working there with a stressful life pattern and a stressful job. You mostly eat from outside and you are more fond of Desi Food or fast food than you are of cooking healthy yourself. And you have not told me if you smoke or not.
Based on these presumptions I would recommend a few measures that you can adopt in your life style to see if your problem gets better. Again, I emphasize to have your heart checked first:
Avoid too much oil in your food. Avoid fried food. Avoid hot food (mirch masala). Avoid eating late at night (eat atleast 3 hours before you sleep). Regulate your sleep cycle to avoid late night sittings and try sleeping at least 6 hours in the night. Ensure physical exercise atleast for 40 minutes at least 3 times a week.
Ensure that you take at least 3 litres of plain water each day, avoid soft drinks and packaged juices. Please ensure that you start your day with two glasses of room temperature water (1st thing out of bed) and slowly increase it to four glasses. Take two glasses of water twn minutes before each meal and avoid water or drinks between meals or till 45 minutes after meals. If you cannot stop it, limit your tea and coffee to maximum two cups a day.
Use cucumbers and lettuce (kheera aur sdalad ka ptta) like medicine and have plenty of salad with meals but not the creamy dressings.
If your heart is clear and all of these measures do not relieve your pain to a significant extent in two to three weeks, then you might need to temporarily add some medicines for your stomach. Otherwise in-sha-Allah you will be fine.

yes you said majority of the things are truth ,yes I was smoking (2 or 3 stick's in a day )but stopped already also I have added already stuff in my diet you said above even I lose about 10 KG in 3 months by doing exercise regularly with medicines below .
As per doctor here in Dubai "there is no superior medicine what you’re having"
