Patient: Arab Khan, 35-year-old Male. History: Status post-cholecystectomy (Gallbladder removed). Chief Complaint: Chronic fluctuation of SGPT (ALT) levels for the past 4 years. Current Status: Liver Enzymes: SGPT reached a peak of 341U/L recently and is currently 312 U/L (as of March 10). Bilirubin and ALP remain normal. Imaging (MRCP - March 12, 2026): Confirmed multiple variable-sized stones in the Common Bile Duct (CBD). The CBD is dilated up to 1.0 cm (10 mm). Other Findings: Grade-1 Fatty Liver, a 3.3 cm simple renal cyst, and a 0.6 cm benign liver lesion. Work-up: Initial metabolic and autoimmune screens (ANA, TSH, Iron Studies) have returned Negative. Requested Intervention: Seeking expert evaluation for ERCP procedure to remove CBD stones and resolve recurrent transaminitis.
