What does the price include?
Hospital packages usually cover the surgeon's fee, the operation theatre and anaesthesia. Ask whether the room, medicines, stent, catheter, tests and follow-up visits are included.
What is urology surgery?
Urology surgery is any operation done by a urologist on the urinary tract or the male reproductive organs. The urologist first examines you and may order an ultrasound, CT scan or urine tests before advising an operation.
Types of urology surgery
Types of urology surgery are as follows:
Endoscopic surgery
A thin camera is passed through the urethra, so there is no cut on the skin. Examples are cystoscopy, TURP, ureteroscopy (URS) and RIRS.
Keyhole and laparoscopic surgery
The surgeon works through small cuts, for example PCNL for large kidney stones or laparoscopic kidney removal.
Shock wave lithotripsy (ESWL)
Shock waves from outside the body break a stone into small pieces that pass in the urine. No cut is needed.
Open surgery
A larger cut is used mainly for very large stones or complex repairs.
Who needs urology surgery?
Your urologist may advise surgery if you have:
- Kidney, ureter or bladder stones that are large, painful or blocking urine flow
- An enlarged prostate that does not improve with medicine
- Blood in the urine that needs a camera test
- A bladder, kidney or prostate tumour
- A hydrocele, varicocele or narrowed urethra
Who should not have urology surgery right away?
Surgery may be delayed or changed if you:
- Have an active urine infection, which is usually treated first
- Take blood thinners that cannot be stopped safely
- Have a bleeding disorder
- Have heart or lung problems that make anaesthesia risky
Tell your urologist about all your medicines and health problems before the operation.
What happens during urology surgery?
- Check-up: the urologist examines you and orders tests such as an ultrasound, CT scan, urine culture and blood tests.
- Before surgery: you are told when to stop eating and which medicines to pause.
- Anaesthesia: most operations use general anaesthesia (asleep) or spinal anaesthesia (numb from the waist down). Small operations such as circumcision may use local anaesthesia, and ESWL usually needs no general anaesthesia.
- The operation: the surgeon treats the problem through the urethra, small cuts or an open cut. A catheter or a ureteric stent may be left in to help urine drain.
- After surgery: you rest in the ward, and the catheter is removed when it is safe. A stent is removed later in a short procedure.
Recovery and results
Recovery depends on the operation. Most people go home the same day after ESWL and return to work the next day. After URS, most people return to work in 3 to 5 days, and after PCNL in 2 to 3 weeks. After TURP, most people leave hospital in 1 to 4 days and return to usual activities in 2 to 4 weeks, but full healing can take up to 6 weeks. Drink plenty of water, avoid heavy lifting for the time your surgeon advises, and go to every follow-up visit. Some blood in the urine for a few days is common after endoscopic surgery.
Side effects and risks
Common side effects are pain, burning when passing urine and some blood in the urine for a few days. A stent can cause discomfort until it is removed.
Less common risks include:
- Heavy bleeding
- Urine infection or fever
- Difficulty passing urine
- Injury to nearby organs
- Stones or prostate tissue coming back, needing another procedure
- After TURP, problems with erections, leaking urine or no semen on ejaculation
Get urgent help if you have a high fever, cannot pass urine, pass large blood clots or have severe pain.
