I’m seeking advice on a case of advanced esophageal cancer to understand what can be done at this stage and the likely outcomes.
Patient Details:
Diagnosis: Moderately differentiated squamous cell carcinoma (HER2-positive), diagnosed early 2024. Treatment History: 10 cycles of FOLFOX5 + Pembrolizumab (stopped due to progression). 10 cycles of Taxol + Pembrolizumab (showed mixed results). Current Status: January 2025 PET/CT shows significant progression in the esophageal tumor, mediastinal nodes, and metastases to the liver and lungs. Next Step: Irinotecan is being considered. The patient also has a history of cardiac issues (two heart attacks, open-heart surgery, and a stent)
Questions:
What are realistic treatment options, considering the progression and cardiac history? How does the prognosis look in terms of quality of life? What steps can be taken to manage symptoms or improve comfort?
