مریض کی طبی صورتحال (Patient Summary) مریض کا نام: غیاث اللہ | عمر: 35 سال تشخیص (بمطابق MRCP رپورٹ): Chronic Recurrent Pancreatitis with Large MPD Calculus. 1. اہم علامات (Presenting Symptoms) وزن میں مسلسل کمی: پچھلے کچھ عرصے سے وزن تیزی سے گر رہا ہے اور شدید جسمانی کمزوری محسوس ہو رہی ہے۔ پاخانے میں چکنائی (Steatorrhea): پاخانہ چکنا اور بدبودار آتا ہے، جو اس بات کی علامت ہے کہ خوراک ہضم نہیں ہو رہی۔ درد کی کیفیت: فی الوقت پیٹ میں کوئی شدید درد محسوس نہیں ہو رہا (Asymptomatic at present)۔ ہضم کا مسئلہ: کچھ بھی کھانے سے جسم کو توانائی نہیں مل رہی۔ 2. رپورٹ کے اہم نکات (MRCP Findings) پتھری کا سائز: لبلبے کی مرکزی نالی (MPD) میں 9.3 mm کی بڑی پتھری (Calculus) موجود ہے۔ نالی کی بندش: یہ پتھری 'Ampulla' کے قریب ہے جس نے نالی کو مکمل بلاک کیا ہوا ہے، اسی لیے نالی پھیل کر 12 mm ہو چکی ہے۔ لبلبے کی حالت: لبلبہ سکڑنا شروع ہو گیا ہے (Atrophy)، جس کی وجہ سے اس کے انزائمز بنانے کی صلاحیت متاثر ہو رہی ہے۔ پلیز اس کا علاج بتائے جو پتھری کا ہے
