Talk to Clinical Dietician on Looking For A Clinical Dietitian's Help To Lose Weight With PCOs

person-avatar

Asking for Self, Female, 29 years old, Islamabad

I have PCOS along with ongoing issues of weight fluctuations, bloating, and water retention, and I’m looking for a doctor who can assess everything together.

Symptoms / medical background:

Weight fluctuates from ~84 kg to ~73 kg depending on swelling/water retention Frequent abdominal bloating and puffiness, especially around waist and hands Malrotated horseshoe kidneys with mild–moderate hydronephrosis Currently on Zyloric 100 mg for uric acid flare-ups Diagnosed with insulin resistance and low vitamin D after full endocrinology workup PCOS diagnosed after ruling out other conditions Thyroid function is normal Autoimmune profile is normal Blood pressure is usually normal, but I occasionally get sudden spikes up to around 180 on some days

I am also looking for a doctor who can guide me with a proper diet plan considering all of these conditions together (PCOS, kidneys, metabolic issues, and fluid balance), not just general advice.

These symptoms are persistent and affecting my daily comfort and quality of life. I would appreciate a comprehensive evaluation and structured long-term management plan.

As a dietitian, I would recommend a comprehensive and individualized nutrition approach rather than a strict weight-loss diet alone. Due to the coexistence of PCOS, insulin resistance, kidney abnormalities, fluid retention, and fluctuating blood pressure, the primary focus should be on reducing inflammation, controlling sodium intake, improving insulin sensitivity, and protecting kidney function. I would advise balanced meals with low-glycemic carbohydrates, adequate fiber, moderate high-quality protein, and proper hydration based on renal status. Processed foods, excessive salt, sugary beverages, and crash diets should be avoided. Regular monitoring of weight, edema, blood pressure, and biochemical parameters is essential. A multidisciplinary follow-up with an endocrinologist, nephrologist, and clinical dietitian is strongly recommended for long-term management and improved quality of life.

Hi, I’m here to help. As a clinical dietitian, I work on personalized nutrition plans for PCOS, insulin resistance, bloating, water retention, and metabolic health issues by considering the full medical picture rather than just weight loss alone.

Prefer a low glycemic index diet such as whole grains, oats, brown rice, legumes, and high-fiber foods.

Include lean proteins: eggs, fish, chicken, lentils, and low-fat dairy in moderate portions.

Increase healthy fats: nuts, seeds, olive oil, flaxseed, and avocado.

Reduce refined sugar and processed foods: soft drinks, bakery items, fried foods, and fast food.

For insulin resistance, eat balanced meals with protein + fiber to stabilize blood glucose.

For vitamin D deficiency, include vitamin D-rich foods such as egg yolk, fortified milk, yogurt, and fatty fish or vitamin d supplements according to doctor\'s suggestion

Maintain proper hydration and limit excess salt if kidney function is compromised.

Avoid excessive protein supplements and high-sodium packaged foods in kidney-related metabolic issues.

Encourage regular meal timing and light physical activity for weight and hormone management.

According to your condition I would recommend a personalized medical nutrition plan, not a general weight-loss diet.Plan should support PCOS, insulin resistance, uric acid control, kidney condition, bloating, and fluid balance together.
Focus should be on low-GI balanced meals, controlled carbohydrates, adequate protein according to kidney function, low-salt intake, hydration guidance, anti-inflammatory foods, vitamin D correction, and avoiding high-purine foods due to uric acid. Because you have hydronephrosis and sudden BP spikes, your diet plan should be made after reviewing kidney function tests, electrolytes, urine report, uric acid, HbA1c/insulin levels, and BP record.
I would advise coordinated care with an endocrinologist + nephrologist/urologist

Dear Patient,
Thank you for providing such a detailed medical history. Based on the information you’ve shared, your situation appears to be more complex than PCOS alone, as several factors may be contributing to your symptoms, including insulin resistance, fluid retention, kidney abnormalities (horseshoe kidneys with hydronephrosis), intermittent blood pressure elevations, and metabolic concerns.

The significant weight fluctuations you describe (approximately 73–84 kg), particularly when associated with swelling and bloating, suggest that changes in fluid balance may be contributing alongside body weight changes. Therefore, weight management should focus not only on calorie intake but also on addressing the underlying factors influencing fluid retention and metabolism.

A comprehensive nutritional assessment would typically include:

• Review of kidney function, electrolytes, uric acid levels, and current medications.
• Assessment of insulin resistance and metabolic markers.
• Evaluation of dietary sodium intake, hydration status, and factors contributing to bloating and edema.
• Review of PCOS-related symptoms and their impact on weight regulation.
• Analysis of meal timing, food choices, physical activity, sleep quality, and stress levels.

From a nutrition perspective management often involves:

• A structured, individualized meal plan aimed at improving insulin sensitivity.
• Emphasis on high-fiber carbohydrates, lean protein sources, and healthy fats.
• Limiting highly processed foods, excess sugar, and excessive sodium intake.
• Careful consideration of protein and fluid recommendations based on kidney function and medical advice.
• Gradual, sustainable weight-loss goals rather than rapid weight reduction.

Given your history of hydronephrosis and occasional blood pressure spikes reaching approximately 180 mmHg, I would strongly recommend continued follow-up with your nephrologist and primary physician alongside nutrition therapy. These symptoms warrant ongoing medical monitoring and should not be attributed solely to PCOS.

Before developing a personalized diet plan, I would typically request recent laboratory results, including:

* Kidney function tests (Creatinine, eGFR, Urea/BUN)
* Serum electrolytes
* Uric acid
* Fasting glucose and HbA1c
* Fasting insulin (if available)
* Lipid profile
* Vitamin D level
* Blood pressure records
* Current weight, height, and waist circumference
Once these results are available, a more individualized nutrition plan can be developed that considers your PCOS, insulin resistance, kidney health, uric acid management, and fluid balance together rather than treating each condition separately.
I hope this helps, and I wish you the best in achieving improved health and symptom control.

I can help with a personalized nutrition and lifestyle plan for PCOS, insulin resistance, bloating, and weight management. Your case seems complex due to the kidney condition and fluid retention, so a detailed assessment would be important before giving specific recommendations.
