Talk to Colorectal Surgeon on Hemorroids Or Fissure

2 years ago while having bowel movements i felt some cute inside my anus and saw fresh drops of blood every time i pass stool i started to search about it and find out fissures but with some lifestyle changes it went away after an year i had bells palsy and i used Deltacortril steroid medicine that disturbs my stomach and after that i usually gets soft stool the other day lose stools and sometimes hard constipated one. So 3 weeks back i got that pain again and this time it was severe, felt someone is pulling the muscles down there burning itching and pain used gtn cream and sitz bath and diet changed i controlled that pain but there is one swollen part you can see in the picture which is the red one it hurts so hurts randomly not while passing stool but after 5 6 hours of it, consulted 3 different surgeons one said its only fissure one said its internal hemorroids and one said its external one that is bothering you. What should i do i don't wanna go for surgery. It doesn't bleed now but just sometimes bearable pain randomly throughout the day
