walaikum u salaam wrwb, ap k teeth py stains hein, black spots stains ko show krty hein or sometimes caries b hidden Hoti hein, you should visit to your dentist , most probably treatment will be scaling and polishing. (not include bleaching in it)

Depends on clinic environment, in our clinic, protocol follow Kia jata hy, proper sterilising instruments / tools use hoty,, cleanliness boht matter krti hy, after each treatment instruments again sterilised hoty hein
