Asking for Brother, Male, 28 years old, Karachi

28 yr old male, obese, smoker for ten years, developed white streak like lesion on buccal mucosa and inner lips more than 6 months ago. The lesions have increased in number since. They are painful only sometimes when they come in contact with teeth. No history of dental procedure. Which speciality needs to be referred? Dermatology, dentistry or some other? Which treatment can be used in the meantime?

The D/D is nicotinic stomatitis causes by heavy smoking, keratosis of area may be due to chemical or thermal irritants, the only treatment of it is to quit smoking And the lesion start to regress within 2-3 weeks with proper cleansing and maintaining oral hygiene.. if on quitting of smoking after few weeks the lesion size remain same than there ll be need of the biopsy. If it persists after month following cessation of smoking need to consult oral pathologists or oncologist

First of all you need to consult a Maxillofacial Surgeon (priority)
Secondly, try quite smoking and do warm water rinses with a salt (if BP issues, then w/o salt) 6-8 times a day.
Can't gave you definite diagnosis but could be keratotic white lesion like lichens planus etc. but you must visit Maxillofacial Surgeon for further evaluation
Or I can refer you.
Seems to be cheek bitting at left, will reduce with warm water rinses.
