She is on wheelchair and not able to walk almost from 20 years. From 2-4 months she is not able to eat properly, high blood pressure , fast heart beat and discomfort.

Walaikum Assalam!

There are three positive findings in this ultrasound report:
1. Fatty liver
2. Smaller left kidney
3. Hydronephrosis in right kidney
In simple words, she has excessive amount of fat in her liver, her left kidney is a little bit smaller than normal, and she has a fluid accumulation around her right kidney.

Although none of these three conditions are dangerous for now, but the underlying disease processes that are responsible for these conditions can cause serious trouble in future. I will need the following investigations, along with a face to face meeting with the patient to have a clear clinical viewpoint:
1. Fasting blood glucose
2. Fasting lipid profile
3. Liver function tests
4. Renal function tests
5. Urine complete examination

Only after having a clear clinical correlation, we can manage/control these processes to stop further damage to her organs, so that she can enjoy a better health in future.

I listed them in the reply.
Liver function tests is a panel of tests offered by the same name by all the labs. It includes Bilirubin, ALT, AST, gGT, and ALP. Likewise, Renal function tests are offered by the same name by labs as a combination of tests which include Creatinine, Urea, and BUN etc.

It's not normal.
Tests needed:
Fasting lipid profile.
Fasting blood glucose.
Liver function tests.
Urinalysis.
Renal function tests.

Also the patient's clinical features are to be reported as part of a definite diagnosis.

LFTS: Serum albumin, bilirubin, ALT, AST, GGT.
RFTs: Serum creatinine and GFR, urine albumin-to-creatinine ratio, BUN.

Get ALT ,HbsAg,antiHCV done . If all are normal then nothing to worry about fatty liver just control weight avoid fatty oil stuff do brisk walk daily . For kidneys please get albumin to creatinine ratio done . Is she diabetic or hypertensive?
