Talk to Medical Specialist on My Cancer Journey & Request For Guidance

person-avatar

Asking For Someone else, Brother, Male, 26 Years old, karachi

I was diagnosed with colon cancer in July 2024 at an early stage. Since then, I have undergone: • Initial procedure with a colostomy bag • Multiple chemotherapy sessions • Radiation therapy • Colon surgery • Immunotherapy

Later, the cancer unfortunately spread to my bladder, and now my kidney function is being managed through urine bags. Chemotherapy is no longer showing positive results.

I’m reaching out to ask if anyone can suggest any further treatment options or advanced therapies that could help in my case.

Any medical advice, expert opinion, or guidance will be deeply appreciated.

Aslam U Alaikum Brother. First of all I am extremely sorry for what you have gone through and what you are going through right now. I simply cannot imagine. May Allah bless you with health, happiness, and patience. Since the cancer has metastasized and is not now responding to chemo-radio therapy and you already have explored and experienced surgical option, my honest straight-to-the-point advice to you is to:
- Pray to Allah all the time Thajud and Duha
- Ask for forgiveness daily. Give anonymous sadaqat
- Have faith that whatever happens it is His plan and He's the best of the planner.

Most likely you should be looking at palliative care, which includes pain management or hospice care. It's better to get the second opinion from a different hospital, from the oncology department, and ask about any new further clinical trials . Once again,May Allah, bless you with all the health and happiness. Brother, I will pray for you and I ask you to do the same.

Aap is post par professional jawab is tarah de sakti hain:
I am sorry to hear about your brother’s condition. Since the disease has progressed despite surgery, chemotherapy, radiotherapy, and immunotherapy, he should be reviewed by a Medical Oncologist at a tertiary cancer center.
Important next steps include:
Reassessment with recent CT/PET scans
Molecular and genetic profiling of the tumor (KRAS, NRAS, BRAF, HER2, MSI/MMR status if not already done)
Evaluation for targeted therapy if a specific mutation is identified
Assessment

Dear Patient,

We are truly sorry to hear about everything you have been going through. We understand how challenging this journey must be.

Since your cancer has progressed despite multiple treatments, it is very important to consult a medical oncologist at a specialized cancer center for a comprehensive review of your case. Your oncologist may consider additional options such as molecular/genetic testing (if not already performed), targeted therapy (if appropriate), immunotherapy reassessment, or enrollment in a suitable clinical trial, depending on your biopsy results, genetic profile, and overall health.

Seeking a second opinion from a multidisciplinary cancer team can also help determine whether any further treatment options are available.

We wish you strength during this difficult time and hope you receive the best possible care.

Aslam U Alaikum. Navigating such an aggressive, multi-modality treatment course is incredibly demanding for both the patient and their family. Based on the documentation provided, the patient is a 26-year-old male who has undergone extensive interventions for colon cancer, including surgery, radiation, chemotherapy, and immunotherapy. The clinical progression to include bladder involvement and compromised renal function is consistent with advanced, metastatic disease (Stage IV). Given that standard systemic chemotherapy is no longer yielding a positive therapeutic response, our focus must pivot to prioritize quality of life and precise diagnostic validation. I recommend advising the family to pursue two immediate, concurrent pathways:Multidisciplinary Second Opinion: Seeking a review at a major academic oncology center is highly advisable. A comprehensive evaluation by an expert tumor board can determine if the patient qualifies for specific clinical trials or novel targeted therapies based on advanced genomic sequencing of the tumor.Early Integration of Comprehensive Palliative Care: This should be introduced immediately, not as a cessation of hope, but as a robust layer of support. A dedicated palliative team will focus aggressively on symptom management, maximizing comfort, managing the complexities of renal diversion, and optimizing the patient's daily quality of life. I wish you best of health. May Allah make it easy for you.
