In short

A haptoglobin test costs Rs 1,260 at Marham Lab, down from Rs 1,800, and Rs 1,440 at Chughtai Lab. MedlinePlus explains the test is done to see how fast your red blood cells are destroyed, and that when red cells are actively destroyed haptoglobin disappears faster than it is created, so the blood level drops. It gives the normal range as 41 to 165 mg/dL.

What is the Haptoglobin test?

A haptoglobin blood test measures the level of haptoglobin in your blood. MedlinePlus explains that haptoglobin is a protein produced by the liver which attaches to haemoglobin in the blood, and that haemoglobin is the blood cell protein that carries oxygen. At Marham Lab the test costs Rs 1,260, reduced from Rs 1,800.

What the Test Actually Measures

This is simpler than most people expect, and it explains why a low result is the worrying one.

MedlinePlus states the test is done to see how fast your red blood cells are destroyed, and that it may be done if your provider suspects you have a type of anaemia caused by your immune system.

It then gives the mechanism: when red blood cells are actively being destroyed, haptoglobin disappears faster than it is created. As a result, the level of haptoglobin in your blood drops.

So haptoglobin behaves like a stock that gets used up. Red cells breaking apart release haemoglobin, haptoglobin binds it, and the supply runs down. A low haptoglobin is therefore evidence that red cells are being destroyed somewhere.

[Reticulocyte count](/labs/tests/reticulocyte-count-test-price)Rs 525Rs 680
[Bilirubin](/labs/tests/bilirubin-test-price)Rs 455Rs 520

Haemolysis is rarely worked up with one test. Haptoglobin falls, LDH rises, indirect bilirubin rises, and the reticulocyte count rises as the marrow replaces losses. A doctor reads the pattern.

The Normal Range

MedlinePlus gives the normal range as 41 to 165 milligrams per decilitre, or 410 to 1,650 milligrams per litre. It adds that normal value ranges may vary slightly among different laboratories, that some labs use different measurements or test different samples, and that you should talk with your provider about the meaning of your specific results.

Read your own report against the range printed on it, not against a number from the internet.

A Low Result

MedlinePlus lists these causes of lower than normal levels.

  • Immune haemolytic anaemia
  • Long term, meaning chronic, liver disease
  • Blood building up under the skin or in other tissues such as muscles, which is called a haematoma
  • Liver disease
  • Transfusion reaction

Two of those are worth separating. In haemolysis, haptoglobin is being consumed. In liver disease, the liver is making less of it in the first place, since MedlinePlus notes the liver produces it. Both give a low number for different reasons, which is why liver tests usually accompany this one. See our [liver function test page](/labs/tests/lft-test-price).

A High Result

MedlinePlus lists blockage of the bile ducts, joint or muscle inflammation with swelling and pain that comes on suddenly, peptic ulcer, ulcerative colitis, and other inflammatory conditions. Haptoglobin rises in inflammation, which creates a real limitation. If someone has both haemolysis and an inflammatory illness, the inflammation can push the number up while the haemolysis pushes it down, and the result can land in the normal range. This is one more reason the test is read alongside LDH, bilirubin and a [blood film](/labs/tests/blood-smear-test-price).

Medicines That Change the Result

MedlinePlus is specific here, and this part is easy to get wrong.

Medicines that can raise haptoglobin levels: androgens and corticosteroids.

Medicines that can lower haptoglobin levels: birth control pills, chlorpromazine, diphenhydramine, indomethacin, isoniazid, nitrofurantoin, quinidine, streptomycin.

Two of those matter a great deal in Pakistan. Isoniazid is a core tuberculosis drug, and nitrofurantoin is widely used for urinary infections. If you are on either, your doctor needs to know before reading this result.

MedlinePlus states your provider will tell you if you need to stop taking any medicine, and that you should not stop any medicine before talking to your provider.

How to Prepare

No fasting is required. MedlinePlus notes only that certain medicines may affect the results and that your provider will advise you. A trained phlebotomist takes a blood sample from a vein in your arm. MedlinePlus notes that when the needle is inserted some people feel moderate pain while others feel only a prick or stinging, and that afterwards there may be some throbbing or a slight bruise which soon goes away. In Lahore the sample can be collected at your home at no extra cost.

Final Thoughts

Haptoglobin is a one-way signal. It drops when red cells are being destroyed, because it gets used up binding the haemoglobin they release. That makes it useful in suspected haemolytic anaemia, and it is almost always read with LDH, bilirubin, a reticulocyte count and a blood film. Two things can mislead it: inflammation pushes the number up, and liver disease lowers production rather than consumption. Tell your doctor every medicine you take, especially isoniazid and nitrofurantoin.

Common questions

A haptoglobin test costs Rs 1,260 at Marham Lab, reduced from Rs 1,800, with free home sample collection in Lahore. The same test at Chughtai Lab is Rs 1,440. No fasting is needed. It may be billed as haptoglobin, Hp or haptoglobin serum, and the price does not change with the name.

MedlinePlus states the test is done to see how fast your red blood cells are destroyed, and that it may be done if your provider suspects you have a type of anaemia caused by your immune system. It explains that when red blood cells are actively being destroyed, haptoglobin disappears faster than it is created, so the level in your blood drops. A low result is therefore the one that suggests red cells are breaking down.

Isoniazid is a core tuberculosis drug and nitrofurantoin is common for urinary infections, so tell your doctor if you take either. MedlinePlus states you should not stop any medicine before talking to your provider.

That pattern needs a doctor to interpret, because haptoglobin can be low for reasons other than red cell destruction. MedlinePlus lists chronic liver disease and liver disease among the causes of low levels, and notes haptoglobin is produced by the liver, so reduced production lowers the number without any haemolysis. It also lists a haematoma and a transfusion reaction. Haemolysis is assessed on the whole pattern of haptoglobin, LDH, bilirubin, reticulocyte count and a blood film.
