Dyspnea and non specific symptoms.
ECG: ST/T wave abnormalities. HCM
Echo: Significant left and right ventricular hypertrophy with apical more significant thickening.
Intra cavity gradient of up to 50 mm Hg

Need to have a detailed history and echo report before decisions about the treatment options. might have to carry a couple of tests like Holter monitoring and stress test if required.
treatment can be percutaneous or surgical depending upon the symptoms and the reports of the investigations
