Does she have piles problem???
What is placental site on Ultrasound... if its rectal bleeding need evaluation from surgeon

U hvnt told about her bowel habits and constipation...r u sure the bleed is from anus...if yes then better consult a general surgeon...risk to the baby if shes bleeding from anus is minimal...but ur wife can end up in anemia...so consultation is essential wth general surgeon and OBG...

-first of all many important points need to be asked/checked to give u better advice and she need examination and investigations as well if not done recently.
-As previous 3 cesearean section she is high risk petient can have placental abnormlities those can be very life threatening if present. So Ned to have detailed Scan by expertise hands to strictly check placental location and its adherance.
-2nd if she has history of constipation and piles even in pregnancy or before pregnancy that also need to be sorted out and then need Precautions to avoid Constipation and treatment for piles.
-have urgent evaluation for both issues as Emergency.
-last cesearean section also need to be done in tertiary care Centre with senior consultant obstetrician better to prefer permanent method of contraceptive (tubal ligation) as this Cesearean could also too risky and dangerous due to previous 3 ceseareans and bleeding.
-it need to confirm that bleeding is anal or vaginal so need examination by Senior obstetrician, surgeon and scan by expert sonologist to evaluate placental position.
