Talk to Gynecologist on Low Bp In Pregnancy

Its 3rd pregnancy(previously two C-sections). 17th week is going, i'm house wife, problem is, when ever i go to market for grocery, suddenly i feel severe dizziness, blurred vision, light headed and legs feel like giving up. I have to sit on foot path or anywhere on road or i have to return home immediately. This happens every time i leave home for market or anywhere else. Is this because of low bp or low Hb levels? What should I do?
