27yo married since 2.5yrs,diagnosd wt PCOS after marriage n conceivd on Clomid 3rd cycle. LMP 21-01-2020 Betahcg doubling in initial tests on alt days 6 wks scan fetus was intrauterine, heartbeat found. 8 weeks March 19, in one ultrasound fetus present, CRL age 7 wks 5 days, no heartbeat, got it repeated on same day, another place n heartbeat was found, I saw and heard it myself, CRL age 7 weeks 1 day. Repeated beta hcg was 59430. 10 wks scan April 2. gestational Sac was 9 weeks 5 days but fetal pole not identified! got it repeated the same day from another place, CRL 0.8cm, no heartbeat, gestational sac age 6 wks 3 days. Beta hcg 91251. No pain, bleeding, cramping etc., still nauseous and vomiting as before. Early pregnancy I had severe lower backache n it still occurs time to time but goes away. I am on folic acid, pregnaessential, glucophage 250mgTDS, Loprin 75mgOD, Duphaston 10mgBD, inj. Clexane 40mgOD, Cyclogest pessary 400mgperrectally HS. Am doctor, different speciality. We r v

Have u done ur transvaginal ultrasound ?

My doc messaged me to wait a week for TVS. taking inj clexane because on 8 weeks scan, the second ultrasonologist said she can see a 1.4cm lesion, might be blood clot or amniotic band, clinical correlation is needed,and that the first ultrasonologist might have mistaken that for fetus and that's why she might have reported no heartbeat. After I told this to my doc she started clexane. I have a history of early miscarriage in July last year.
Is there any possibility this might be molar pregnancy (partial mole)? :(

I repeated my hcg today and it is 55911. If it's already a miscarriage shouldn't it be less than this? I read at various places that it declines after ten weeks. Ultrasound is due after two days. What should I expect?

Sorry to hear about these complications have you consulted a competent specialist, you are on too many medication because if it is a non viable pregnancy than you should not be on all of these medication better visit your consultant and please do as advised if you will visit too many doctors they will start you on unnecessary medication. First of all we need to do proper dating of pregnancy then we can diagnose how many weeks you should be , repeated scans with different operators of variable qualifications and experience will complicate the situation if you want I can help

As the gestation sac is still in uterus (implanted )the level of HCG will continue to rise, the HCG levels in Moler pregnancy will rise exponentially the repeat Beta HCG level 48 hours apart will diagnose that also the ultrasound features are not suggesting Molar pregnancy some time it could be a partial molar pregnancy in that case the HCG levels will be very high but typical mOlar feature might not be there on ultrasound

At that sac size we should see a foetal pole , if whatsoever they saw was more than 7mm size with no cardiac activity it goes in the favour of missed miscarriage

At 48mm Sac size there should be a definite fetal pole if no fetal pole than it is Blighted ovum

TVS has not been done. My doc advised to wait for a week and then go for it.
Clexane was started because on 8 weeks scan, the second ultrasonologist said there is a lesion of 1.4cm, might be a blood clot or amniotic band, which might have been mistaken by first ultrasonologist as fetus and hence she might have reported no heartbeat. After that I was put on clexane.
I had an early abortion in July 2019 hence so many medications.
Is there a possibility this is molar pregnancy? Partial mole? 😔
