heart working 20% one wall is leak. In April, a blood clot went to leg, and then the vascular doctor operated at CMH hospital. After that, condition improved. But now, 4 months have passed, and having a lot of breathing problems. They admit to the hospital, but they don't agree to any kind of surgery. Medicines aren't making a difference, and condition is worsening. Is there any solution that can be found?

What is age of patient? If they aren't doing surgery, it's because it can have complications in an elderly patient with poor ejection fraction of 20 percent. Main treatment is medical with Intravenous diuretics and other medications which they are already giving now.
This is case of dilated Cardiomyopathy/ Congestive heart failure. Need proper diagnostic work up & management . Surgery is not advisable.

Coronary artery disease with resulting myocardial infarction and the loss of functioning myocardium is the most common cause of heart failure. Hypertension is also an important cause of CHF. Since both of your patients are on diuretics, it is important to assess mineral status due to drug induced nutrient depletion, especially magnesium. Potassium levels can be checked on a CMP, however, you will need to order an RBC magnesium to assess nutrient status of magnesium. An RBC nutrient and toxic element test will be extremely helpful, especially if you suspect heavy metal. Cadmium has an affinity for the kidney and can lead to hypertension.
For patients with stable heart failure, the approach must be multidisciplinary. Identifiable causes of heart failure include: hypertension, CAD, valvular disease, alcohol abuse, diabetes mellitus, chemotherapeutic agents, chronic anemia, hypothyroidism, and toxic exposure. The following is a list of integrative treatment recommendations to consider:
1. Smoking cessation
2. Treat comorbidity
3. Dietary prescription - organic fruit/vegetables, fiber, essential fatty acids, foods high in B-complex, use olive oil (no transfats), low sodium foods
4. Weight reduction
5. Graded exercise
6. Antioxidant, Co-enzyme Q10 and carnitine status are extremely important to assess in patients with CHF. An Organic Acid test is a must.
7. Hawthorn: increases coronary artery blood flow and enhances pumping efficiency.
