Talk to Internal Medicine on INR Level Is Low, 2 Metallic Valves Patient

My father is on Warfrain medicine from so many years, he maintains his dosage by checking INR Level which he has to maintain 3 to 3.5, but recently he got severe malarial attack which effected everything like HB, Platelettes, bp etc. INR was shppted up suddenly to 14, then admitted hospital doctors gave him vitamin K injection. after which INR Decreasrd to 1.3 suddenly, then started normal dose of Warfrain 5mg , then 7.5mg, INR went up to 2.5 then again its Decreasrd to 1.7, 1.9 which is Low as he is cardiac patient.
He is not taking green leafy vegetables as we know it contains vitamin k, What to do kindly guide.

also note this Dr. sb, that when he had malaria his INR level went to 14, hospital doctors gave vitamin k injection after which INR dropped to 1.3, then it increased little bit till 2.5 now again it's dropping and continuously it's 1.7,1.8 that's making us worry. how to increase it now

sir, the tablets come in 5 mg only by shagon company. So only 5,7.5 or 10 mg is possible to take.
From yesterday he has started to take clexane 12 hourly injection along with 7.5mg , is it ok? or should warfrain does also be increased more?
