71-year-old female patient admitted due to infection and septic shock. She had low blood pressure and shortness of breath and is currently on oxygen support. Ultrasound and CT scan were done. Chest ultrasound showed minimal pleural effusion (~50 mL) only. No current pain. Vitals are mostly stable. Main concern: Since yesterday, the patient has developed continuous confusion and irrelevant speech. She talks off-topic persistently. Speech is clear, but the content is disoriented. This behavior is new and was not present before hospitalization. She is otherwise alert, recognizes people, and routine behavior is mostly normal except for continuous confusion. Concerned about delirium, post-sepsis encephalopathy, hypoxia, electrolyte imbalance, or medication side effects. Kindly advise further evaluation and management.
the patient is in acute confusional state/ delirium. review fresh CBC, serum electrolytes, urea, urine output, hydration status. Rule out metabolic encephalopathy first and then neurological organic causes via CT if required.
