Asking For Husband, Male 43, Allama Iqbal town
now a days ramadan here, after fasting of 6 to 7 hours severe headache started in front half head part, feels like fever, but after 1 hour of iftari it started to heal and gradually in 4 to 5 hours he feel comfortable. what symptoms it may, plz guide
Check HBA1C and how much tea/coffee was he driking before Ramadan? Any symptoms before Ramadan?
Dehydrated , hypoglycemia, caffeine withdrawal, needs workup and lifestyle modification
Hyper acidity also leads to such symptoms .it can be treated with proper diet and medicine.its stomach issue
Could be due to disturbance in blood sugar level, blood pressure level, sleep disturbance, stress, caffeine/nicotine withdrawal symptoms, or some other issue. You can advise him to get a good 7-8 hrs sleep, check blood sugar and blood pressure as needed, use complex carbs and slow to digest foods in sehri, avoid stress, gradually cut down on tea/coffee/cigarette (if using, abrupt cessation can cause these symptoms); if still not better then consult a medical specialist. Drink lots of water.

This pattern is quite typical and not likely something very dangerous, but it still needs a structured answer.
Most probable causes
Dehydration
Fasting for 6–7 hours (especially in warm weather) → reduced fluid intake
Headache improves after iftar → strong clue
May feel “feverish” without actual fever
Hypoglycemia
Low blood sugar during fasting can cause:
Frontal headache
Weakness, irritability
Relief after eating supports this
Caffeine withdrawal
If he usually takes tea/coffee and stops during fasting
Headache improves after evening intake
Tension-type headache
Frontal, pressure-like pain
Triggered by fatigue, dehydration, or stress
Less likely but important to consider
Migraine (if severe, throbbing, light sensitivity)
Sinus-related headache (if nasal symptoms present)
What to advise (practical management)
During Ramadan:
Ensure good hydration between iftar and sehri
Add oral fluids + electrolytes
Avoid excessive caffeine withdrawal (gradual reduction)
Take balanced sehri (complex carbs + protein)
Adequate sleep
Simple medication (if needed):
Paracetamol at iftar is reasonable
Red flags (need evaluation)
If any of these are present, don’t ignore:
Persistent headache even after eating
Vomiting, visual disturbance
Very high blood pressure
First-ever severe headache
Neurological deficit
