Asking for Brother, Male, 31 years old, Karachi
Dear Sir/Madam, my younger brother had accident on bike in Oct-2019 & his leg bone break then doctor advise to attach rod but due to feeling pain because of rod is 2 inch long in size & during walk he feels pain, so my brother consult with doctor to remove this rod then doctor agreed to remove this rod, & last month 18-Feb-2020 doctor removed his rod & bone is already jointed but on 27-march-2020 he is walking at home and suddenly he feels current in his leg at rod attaching area & cant walk anymore then we took him to the hospital & done x-ray (X-ray attached) & after x-ray doctor told that his bone already break at same area, & doctor told to again attach same rod in same area for forever, Kindly please advise us what to do, & where we will decide to done his operation again.
Need to review the pt and see why the bone fractures again. Need some tests to confirm the reason. Regards Prof M A Wajid
What about skin condition? If all is OK, and patient is not willing for surgery, Can be managed conservative with pop

Maybe some underlying metabolic disease leading to pathological fracture everytime.

His rod is removed too early and now he has developed hypertrophic nonunion. Best is to get surgery again with reamed nail in dynamic mode.
need to assess the patient condition, soft tissue status .. might need rod for early mobilisation

bone didn't heal yet or break new. He shall not put weight on this leg for 6 to 8 weeks then get x-ray yo decide. Thanks
Consult any good experienced Orthopeadic surgeon in your area: My plans about this patient would be : First to rule out infection ( by inspection and some blood tests ) secondly if there is no infection will go for exchange IM Rod a technical strategy and the best option to bring healing of this fracture and if there is infection then I would go for Antibiotic Rod first . I. This case Rod was malplaced and malpositioned .
kindly give bone chance to heal in pop cast and regular follow-up but after getting him checked, if no progress then can go for optn.

bone is breaking again. investigating the reason is important.
Needs workup for non union. Meanwhile brace or splint can be given and weight bearing should be avoided.
There is no need for surgery. He has to walk with stick and put full weight on it
Clinical and lab assessment is necessary
And aslo send some previous radiographs with nail and after nail removal
