if skin gets necrotic or slough forms then may need debridement. otherwise wait and do antiseptic dressing. and time will tell what else your brother may need.
give syp nezkil suba sham. syp flagyl suba sham in addition to syp brufen and some multivitamin syp like amvit.

Just follow PIMS if required they will do proper management. Usually children r good healer and dont require any operation. But best is follow advise of a center where they have burn center and paediatric/plastic surgical department.
