Dear Doctor, My son (2.5 years old) sustained a burn on his hand about two weeks ago. Alhamdulillah, there has been significant improvement; fever and swelling have reduced. Current treatment is ongoing, which includes:
Cleaning with Normal Saline 100ml and Povidine twice daily
Application of Dermazin cream (special burn cream)
Sofratulle dressing (special wound dressing) change
Crepe bandage 4x4 cm (to secure the dressing) together with a splint/
wooden support to keep the hand straight
Use of heat bandage Mild exercises
However, when the dressing and wooden support are removed, the wrist bends inward automatically, while fingers have some movement.
Please advise: How long should the current treatment continue?
Any changes required in treatment?
Is the heat bandage safe, or could it harm muscles/nerves?
What measures can prevent the wrist from bending inward?

Al depends on wound condition, location and number of joints involved. Visit any burn specialist for proper examination and guidance.
it is better to get expert opinion as hand burn could end into lifelong disability.

Managment Depending upon type , extent and depth of burn
Limb Elevation is must .
keep wrist Extended.

your concerns are genuine.
we dont know current status of hand, its better to visit a burns expert for further management.
generally such patients need regular follow up for longer period of time untill hand become near to functional.
The treatment you have mentioned is adequate. I would suggest change of dressing every other day to twice a week. Next time you change dressing please take a picture (focused) and share it on forum

Assalamualaikum Doctor,
We are sharing the latest fresh picture of the wound. Alhamdulillah, the wound has improved significantly. Due to the child being very active, we try to change the dressing every other day, but he often removes the bandage himself, so we have to redo the dressing.
Currently, the fingers have some movement but not full strength, and the wrist bends inward when the dressing is removed.
Kindly guide us whether we should continue the dressing at this stage or leave the wound open. Also, should the fingers be kept open now?

If you can bring in for consultation that would be better. Usually night splinting and massage therapy and physiotherapy is to be continued for 3 to 6 months but it depends on individual patient. Please bring in for consultation for proper guidance.
