There are various possibilities depends on onset is acute or chronic if it's slow to develop and associated with wheeze chest tightness and increasing symptoms on walking then as mentioned above it could be COPD .you can consult a general physician prior to consulting pulmonologist so he can arrange basic investigations and work up before you could be referred to appropriate specialist.

My uncle has been smoking for 25 years. Docotors of Shaikh Zayed Hospital diagnosed CKD + LRTI and referred for Bronocopy Test, but patient is not willing for doing this. He feels daily fever 100 and + between 2-3 hours. Where he should consult?
