online consultation is available through marham
1. Swallowing Safety (Most Important)
Because she coughs while drinking → risk of choking.
Do this:
Give thickened liquids (not plain water)
→ use soups, yogurt drinks, or slightly thick water
Always keep her head slightly elevated (30–45°) during feeding
Feed slowly, small spoon at a time
After feeding, keep her sitting/upright for 20–30 minutes
❌ Avoid:
Thin liquids (plain water, tea)
Using straw (can cause choking)
?️ 2. Speech Stimulation (Low Effort)
Even if speech is slurred, stimulation helps brain activity.
Daily (10–15 min, 2 times):
Ask her to repeat simple sounds: “Aa”, “Oo”, “Ma”
Encourage slow speaking
Use yes/no questions (blink, nod if possible)
Play her favorite recitations or voices → she may try to respond
? 3. Oral Muscle Exercises
(Do very gently)
Lip closing: ask her to press lips together
Tongue movement: move tongue side to side (if possible)
Light cheek massage
? This helps both speech + swallowing
?️ 4. Positioning & Physical Care
Since she is bed-bound:
Turn her side every 2–3 hours (prevent bed sores)
Use pillows for support
Keep mouth clean (oral hygiene daily)
? 5. Diet Plan
Since she is already on semi-solid:
Give soft mashed foods (khichdi, mashed banana, yogurt, dal)
Avoid dry, hard, or crumbly foods
Ensure nutrition + hydration
? 6. Medication Awareness
She is taking Carbamazepine for Trigeminal Neuralgia
? This can sometimes cause:
Drowsiness
Reduced alertness
So always feed when she is fully awake
❤️ 7. Emotional Care
Talk to her daily, even if she doesn’t respond
Play Quran, duas, or calming sounds
Gentle touch and reassurance matter a lot
⚠️ When to Seek Urgent Help
Frequent choking
Food coming out of nose
Fever after swallowing issues (risk of aspiration)
Sudden worsening of speech
⭐ Important Note
This plan helps at home, but honestly, she needs a Speech & Swallow Therapist (SLP) visit—even once—to guide properly for dysphagia therapy.
Has the patient got evaluated for swallowing like FEES? Coughing after Intaking water is indicating aspiration which needs evaluation and rehabilitation ASAP.
And yes, I can offer home visits
I understand your situation—this requires a very experienced neuro SLP (speech therapist), ideally one who can handle dysphagia (swallowing), dysarthria, and bed-bound care.
