I had brain tumor surgery in March 2024 and the tumor was attached to the facial nerve, resulting in immediate left-sided facial palsy and numbness. I have significant facial asymmetry on left side of my face; specifically, my masseter muscle and the temporalis muscle appear dead or atrophied, causing my face to look sunken, as you can see in the photo. Also, when I open my mouth, my jaw is slightly deviated towards the left side. After a year with no improvement, I am seeking a plastic surgeon's opinion: what are my options now to restore symmetry, and is a nerve transfer recommended?

n cases of long-standing facial paralysis (more than one year), the treatment options become more limited due to irreversible muscle atrophy and neural degeneration.
When both the facial nerve (VII) and the motor donor nerves such as the masseteric or temporalis branches of the trigeminal nerve (V) are non-functional or significantly atrophied, nerve transfer procedures or dynamic muscle reanimation are no longer viable options. In such situations, the target muscles have lost their ability to respond, making reinnervation ineffective.
In these scenarios, facial symmetry can be improved through static and volume-restorative procedures, such as:
1.Fat grafting to the temple, cheek, and midface to restore volume deficiency or
2. Dermal fillers to improve contour and soft-tissue symmetry or
3. Alloplastic implants (e.g., temporal implants) for more stable volume correction
However, it is important to understand that dynamic asymmetry during facial movements, such as asymmetry while opening the mouth or smiling, may not be completely correctable, and a certain degree of asymmetry may need to be accepted.

By looking at your picture I agree there is mild asymmetry. I doubt if your facial nerve was completely transected during surgery. In my view you should expect further improvement in 12-24 months. You should avoid any invasive procedures. The option to improve symmetry in the interim period is facial filler or fat graft. These are both temporary solutions. Patience will pay off in the long run.
