I was first diagnosed in August 2023 with left-sided varicocele only. In 2024, a repeat ultrasound showed bilateral varicocele (mild right, mild–moderate left). On 9 Nov 2025, my latest Doppler ultrasound shows bilateral Grade II varicocele. I have attached the report recent doppler ultrasound and last year august semen analysis report. For the last several months, I experience recurring severe pain every 1–2 weeks, often triggered after mental stress or negative thoughts, and within 1–2 hours I develop intense scrotal pain, swelling, redness, warmth, and pain radiating to the hip. The pain becomes worse on standing or coughing and improves when I lie down. I request your guidance on whether I should consider microsurgical varicocelectomy and what the best long-term management plan would be. Thank you sir
Thank you for sharing your reports and symptoms. Your Doppler shows bilateral Grade II varicocele, and the pattern of pain, worse on standing and relieved by lying down is typical of symptomatic varicocele. Since the pain is now recurring despite conservative measures, you are a reasonable candidate for a microsurgical sub-inguinal varicocelectomy, which offers the best long-term results with low complication rates.
