Talk to Urologist on Erectile Dysfunction And Premature Ejaculation

For the last 2 months I have been experiencing erectile Dysfunction and Premature Ejaculation. I have been married for more than 2.5 years having one baby.
However, for the last two months I am facing issues in maintaining erection during sex and Premature Ejaculation. I consulted with one doctor but I am not comfortable with the treatment. I have attached my recent lab reports and want opinion on if there is any physical deficiency which has led to this problem. Important to mention that previously there were no problems in sexual activity.
