Talk to Urologist on Erectile Dysfunction And UTI

I 26m have erectile dysfunction since the last 7 years and UTI last 3 years, and during all these years i still masturbated almost everyday to the point that i don't have an urge for masturbation at all now since last a year or so,.....my penis is shrunken in size completely, i haven't ejaculated through masturbation since the last 3 years, and i get night fall once in 3 or 4 months, releasing a very tiny amount of semen. I've an urge for urination very frequently, i feel burning during urination, my urine colour is very dark yellow, and my lower abdomen is always slightly swollen, my lower back hurts, whenever i lift something slightly heavy, the pain in my lower abdomen worsens and i've a strong urge for urination...... I know i'm in a worse state right now, but Is there any slight chance of me getting treated and normal one day?
