prostate enlargement is called BPH(benign prostatic hyperplasia)
first we give medicine trial for atleast 1 month. if the patient responds to it then that medicine is continued for 2 or 3 months or even lifelong with followups every month. If the patient does not respond to medicine or if there is not improvement in the symptoms at all, then we go for surgery either TURP(Transurethral resection of the prostate) or VLAP(visual laser ablation of the prostate)
OR
if the patient is already taking medicine for BPH and now has stopped responding to that medicine, then we also go for surgery
