My father is having severe urinary incontinence even though it's been 1 and a half year since his turp and tvp surgery for bph. I want to ask what could be the next best possible solution that is available in Peshawar

April 2023, my father had urinary obstruction due to prostate enlargement according to medical reports(bph) to more than 70 GM resulting in hydronephrosis and hydroureter and also had chronic cystitis. Then via catheter his bladder was which had 700ml of urine. After a week he had tvp and after procedure he still had some urinary retention and also infections developed over half an year and also his lower leg and foot areas skin became really dark and hard and some kind of dermatitis and also he says his legs became really heavy also has a burning sensation in his feet and legs to this day and he had constipation before the surgery but after it became really worse and now he can't defecate with our having to use laxatives.
So he really became weak , his legs couldn't work properly and knees starting bending on its own when walking and the urinary retention and also the stress or overflow incontinence and all the infections so he had another procedure and this time it was TURP on 13 September 2023. After that the urinary retention and infection stopped after use of antibiotics and other medicines. Also the anticholinergics was not doing good to his nervous system and gastrointestinal system and his constipation got worse and worse . His urinary incontinence remained the same but his walking got a little better with strict diet control of having as less carbs(per my uncle's advice who is a neurophyscian) as he could then my neurophyscian uncle advised to start a Tb course . He used that for about 30 to 40 days but then my father started his normal carbs containing diet but I don't if it was after that course or the more carbs less protein diet that caused him to become weaker. Then per my uncle's advice along with neurobion injections folic acid and magnesium liquid drops in water we gave him thyroxine for a month or more.
But after every thing his severe urinary incontinence, constipation, burning sensation in feet or some times whole body, dizziness or vertigo when he stands or walks and dermatitis in lower legs. His knees don't extend properly which makes walking even harder.and if it's relevant he had a minor bike accident almost 3 years ago but he didn't follow up properly with the doctor and i think his leg was injured. I still give him neurobion injections and thyroxine I have stopped but he has really become hopeless and doesn't believe any doctor can help him so no matter how much we try he won't go to any doctor and now he doesn't and can't leave this house and it has been almost half a year since he went outside the house.
