Asking for Self, Male, 28 years old, Lahore
Assalam o Alaikum respected doctors…! Please help us, we are too worried and every passing day is killing us from inside... :(
Got married 10 months and 22 days ago. Just after 3 months of marriage, I've been diagnosed with Low Sperm and Motility. Visited many doctors since then, but there is no improvement. I've been taking the following medications; YTig + Evion : 1.5 Months Some Unani Medicine + Evion : 1.5 Months Gunadil-F + Bifera (Iron Supplement) + Evion : 3 Months and now recently a doctor just gave me an injection (I don't know what it was) and prescribed me L-Arginine for 1 month + doxyn for 8 days only, he said after 1 month of taking these medicines I can go for a sperm analysis and u/s testis. I'm still taking L-Arginine.
I'm also attaching all my reports during this time. You can check them all with date/time mentioned on them.
We both are so worried as no improvement has been shown in almost 1 year. Please help me and guide me for a solution.
Jazak Allah

Dears,
I know already how to book an appointment... All I want to hear from you is to elaborate the possible root cause of the problem and available solutions we can try... Please stop asking me to book an appointment I already know how to do it... at least show some guts so that you don't have to ask for appointment... make me do this by elaboration... I really don't know what's going on here... Marham team should take a serious note about this
Go for Ultrasound testis..to find out he cause of low sperms..
& still u can go for fertility techniques like IUI & ICSI
Dont be sad and dont loose hope
Semen reports u have shared shows that in between ur semen parameters got better (may b due to medicines), but then again those parameters started declining. And the latest report shows the same thing
In semen analysis, we look for 3 things: sperm count, progressive motility and morphology.
All these parameters in ur latest report are decreased. And this is called OAT syndrome. Usually it is due to varicocele (engorgement of testicular veins) and it is picked on scrotal ultrasound
The dr who prescribed u these medicines and then asked u to take it for 1 more month and then go for tests.. this a wrong approach
U dont have to wait for 1 month. As your semen analysis shows all 3 parameters are declined, so immediately go for DOPPLER ULTRASOUND SCROTUM so that to look for varicocele. Suppose if ur ultrasound comes out normal, then we will go for ur hormone tests that includes SERUM TESTOSTERONE, FSH, LH, PROLACTIN.
In case of male infertility we go for these tests (ultrasound and blood tests)
If ur ultrasound shows varicocele then u have to go for surgery of varicocele as it is treatable only thru surgery. Suppose if ur ultrasound appears normal but ur hormonal tests show some defect, then we will go for ur testicular biopsy to check whether ur testes are working properly or not and in the biopsy report it is mentioned that whether there is any defect in the sperm producing cells or not, and a score is calculated called Johnson score that indicates that whether the couple will benefit from IVF or not
And suppose all ur tests appear normal i.e. ultrasound and hormone tests both appear normal, then it means naturally u have defected sperms... in that case u have to take gonadil F for 3 months which u r taking and then go for IVF after 3 months and have fresh semen analysis before going for IVF
In my opinion u should only take Gonadil F and stop taking other medicines and immediately go for ultrasound.
This is the correct approach we follow in case of male infertility
