I am truly sorry to learn about your daughter. Based on the information you have shared, including the echo reports, it seems a catheter-based procedure using a plug or mesh seems viable and could potentially close the ASD with least trauma hence should be the 1st choice of therapy. However, a comprehensive evaluation is necessary, preferably involving 3D echo, or if 3d echo is not available alternatively, a standard echo or MRI to assess the rim's integrity and dimensions where the device will be placed. I believe the advice from AFIC is sound, and proceeding with device closure appears to be a reasonable course of action. Following the procedure, she will likely require blood-thinning medication for a few months and regular echo follow-ups to monitor the device and the mitral valve. Hopefully, as it is anticipated that any existing Mitral valve leakage will improve or, at a minimum, stabilize as the right-sided heart pressure normalizes post-procedure.
