Two of the dozen other nail changes in a diabetic patient are sunungual hematoma(bleeding under nails) and fungal infection. Also spontaneous injuries can occur in diabetic patients so pay attention on diabetic neuropathy that occurs in diabetic patients (especially if they have diabetes for many years) and for the treatment of fissures between nails and sole of the foot use anti fungal cream such as clotrimazole cream

Sensations are intact. Canesten has been started and now its improving. Is there any need for further treatment like usage of any topical antibiotic ? Or Canesten is enough ?

If use of anti fungal cream giving result then its enough but if few lesions especially under the nail or near nails do not improve then u can add anti bacterial cream containing fusidic acid salt. The lesion in between or under the sole are fungal infection so keep using anti fungal cream in them for 2-3 weeks or more.
