Talk to Dermatologist on Please Guide Me,

i am now in dubai,i took medicines according to my dermatologist for 3 months but improvement was 50 percent,,now i am out of the country ,,i go to doctor here,,she says i cant understand what your doc in Pakistan has written ,,i am attaching a photo on which there is diagnosis report of what kind of skin disease is written, doc in dubai wants to know because she cant understand,,i have also tried to contact that doc in pak,,but they are not responding,,please ,,any dermatologist here help me ..tell what kind of skin diseases are written ,,please help me am waiting
