It's extremely difficult the diagnose the skin condition but it looks like to be an eczema. Since only one palm ie Right palm is affected it may have been brought on if she is handling something regularly and very often with her right hand. Maybe pan handle in the kitchen. If that is the case then the eczema won't go away unless an until the offending thing is avoided.

Pls try to control diabetes and take an appointment for proper treatment
this is Hand Eczema and has several causing factors.
Hand Eczema doesnt cause pain until it is secondarily infected.
Pain can be due to Vitamin D deficiency.
I suggest her to Take Vitamin D supplements for One Month and if still pain not get settle then consult Orthopedic surgeon ot rheumatologist.
