Subject: Request for expert opinion on Revision Surgery (Stapedotomy)

Dear Sir, I have been suffering from bilateral otosclerosis since 2018 and have been using hearing aids. I have been advised to undergo stapedectomy in Pakistan. In 2022, I recently underwent Stapedotomy for the right ear in December, 2024.

Highlights for Right Ear Stapedotomy: • Test result before surgery: 52 dB • 8 days after surgery: 45 dB • Following consecutive checkups for two months 2 months: 42 dB • Observing low cracking-like sounds while yawning, swallowing something, or opening the mouth for burping or belching. It feels like the ear is open, and I hear very clearly for a moment. • Experiencing no pain.

In addition to the abive,

I was wondering if this is inheritant in South Asians having thick bones or require longer prosthes. I would be grateful for your expert opinion whether I should go for a revision surgery or not. Further, shall I use hearing aid in the oprated ear (right ear) as I still feel I need a hearing aid for the right ear as well.

1. Please share your Audiogram and Tympanometry results before and after surgery?
2. Crackling sounds could be related to Eustachian tube dysfunction and Temporomandibular Joints. X-ray TM joints will be needed.
3. Prosthesis length is measured at the time of surgery just before insertion. Generally, 4.25 mm is mostly used, however length ranges from 3.75mm to 5.25mm. Furthermore, shaft diameter is also important and commonly used are 0.4mm and 0.6mm.
