Talk to Hematologist on Autoimmune Haemolytic Anaemia

Asking for Mother, Female, 67 years old, Khairpur

My mother has autoimmune haemolytic anaemia and the doctor asked for Rituximab injections, she got 10 packed cells transfusion since December'24 and got admitted in hospital for several times. Recently her blood platelets got dropped to 14 thousands. Her blood culture report done on 3rd August showed that she has Typhoid (Salmonella Typhi) She is on Prednisone and is suffering from the side effects of those and now she can't walk. My Question is, If the typhoid was the cause of AIHA? Pls guide what is the treatment besides Rituximab injections as those are very expensive and we can't afford them.

First-line cheaper steroid-sparing agent: Azathioprine (50–150 mg/day depending on weight).

If refractory → consider Cyclophosphamide (oral) or splenectomy if feasible.

Also optimize: folic acid, vaccinations (pneumococcal, meningococcal, H. influenzae), and infection prophylaxis if needed.
