Talk to Hematologist on CBC Report Analysis: Normal Hb (12.4) But Low MCV (70.9) And MCH

person-avatar

Asking for Self, Female, 21 years old, Lahore

I'm Akasha . In my CBC report, my Hemoglobin is 12.4 g/dL (normal), but my MCV is 70.9 fL (low) and MCH is 20.9 pg (low). An online doctor has advised further testing (Iron Profile and Hb Electrophoresis) costing 6,000 PKR. Since I do not have any severe symptoms, please guide me if these tests are absolutely necessary right now or if I can manage this initially through an iron rich diet. I am attaching my report below.

Your hemoglobin is normal (12.4 g/dL), but your MCV and MCH are low, indicating that your red blood cells are smaller than normal. The two most common causes are iron deficiency and thalassemia trait. An iron-rich diet alone cannot distinguish between these conditions and may not be appropriate if you have thalassemia trait. Therefore, I recommend at least an iron profile (especially serum ferritin). If iron deficiency is excluded, Hb electrophoresis should be done to look for thalassemia trait. These tests are important but are not an emergency if you are otherwise well. Please avoid taking iron supplements unless iron deficiency is confirmed.
