A young female with heavy menstrual bleeding, passage of clots, easy bruising, and microcytic hypochromic anemia (low Hb, MCV in the 60s). Pelvic ultrasound is reported as normal. The clinical picture suggests iron deficiency anemia secondary to menorrhagia; however, an underlying bleeding disorder should be excluded.
Recommended investigations: • Serum ferritin (± iron studies) • PT/INR • APTT • Fibrinogen • Platelet function studies/bleeding disorder screen if available
Further evaluation should be guided by the results of the above tests.
