Asking for Self, Female, 24 years old, Hyderabad
Mujhe physical kaam ya zyada der khare rehne par shiddat se thakan aur chakkar aate hain. Normal daily routine work bhi nhi ho pata. Shadi ko 10 months ho gaye hain. Shadi ke dusre din se hi chakkar shuru hoe jo ab tak barqarar hain. Us time nausea bhi Hota tha but vomit nhi hoti thi. 1st period delay hua, HCG detect nhi ho rahe they but symptoms they, phir Kuch dino bad sirf 2 days heavy clotting hui. Jab husband ke sath rahi to phir periods mein clots aaye aur BP 40 tak gir gaya, jisse extreme shivering hoti thi. Pakistan aa kar gynaecologist ko dikhaya, unhon ne urine, thyroid, CBC aur abdominal ultrasound and whole abdominal ultrasound karwaye. Us waqt right side par severe abdominal pain tha even I can't bend. Thyroid/urine normal thay. Ultrasound mein ovaries bulky, eggs immature aur kidney mein halki swelling nikli. Dr ne sirf folic acid likhi aur kaha ke main healthy hoon, chakkar aana koi masla nahi, cal c le liya karoon JB bhi feel ho. Lekin still meri physical activity aur workout bht mushkil hojatey hain energy drain honey ki waja se even Mai market bhi nhi ja paati. I read report of CBC twice by myself Hematocrit, MCV aur MCH normal se kam hain, jabke RBCs zyada hain. Kya ye values meri thakan aur chakkar ki wajah hain? Meri routine disturb hai. Please suggest.. hairfall bhi hai kaafi and skin color pale honey lga hai.

I miss that point 4 weeks k baad and 5 weeks SE pehley heavy periods start hoe they UN 2 times severe pain tha. Previous Dr said it might be miscarriage but ap ko blood test krwana chahiye tha ab koi confirmation nhi to hum isey pregnancy nhi keh skte. I learned about chemical pregnancy. To kya ye chemical pregnancy hoskti thi ?
Dear, I have gone through your history carefully. Your CBC shows a mild anemia (Hb 11.5 g/dL) with very small, pale red blood cells (low MCV and MCH), but your RBC count is relatively high. This pattern is more suggestive of a thalassemia trait than iron deficiency alone. However, thalassemia trait by itself usually does not cause such severe tiredness, dizziness, poor exercise tolerance, hair fall, and marked weakness.
Your symptoms deserve further evaluation. It is important to check your serum ferritin (to rule out coexisting iron deficiency), Hb electrophoresis/HPLC (to confirm or exclude thalassemia trait), and, if indicated, vitamin B12, folate, and vitamin D levels. Since you have also had irregular bleeding, severe abdominal pain, and ultrasound findings related to the ovaries, a follow-up with your gynecologist is also advisable.
Please do not start taking iron supplements unless iron deficiency is confirmed, because if you have thalassemia trait without iron deficiency, unnecessary iron is not beneficial. Once these investigations are available, the exact cause of your symptoms and the appropriate treatment can be determined. I wish you a speedy recovery.
