His AFP is >1000, so he is not a candidate for liver transplant. Also, needs to discuss with radiologist if the portal vein thrombosis is tumor related or bland. Need to know performance status, liver functions (LFT, Albumin, INR, child and MELD scores to determine if he will tolerate any locoregional treatment (TACE or TARE) or systemic immunotherapy to shrink the size of tumor and reduce AFP to downstage the tumor for liver transplant.
