Talk to Internal Medicine on Abnormal Bleeding From Tiny Scratches

person-avatar

Asking for Self, Female, 29 years old, Islamabad

I have a history of kidney related issues, high blood pressure and water retention too...a few Months ago my kidneys were showing sigs of stress, water retention, gout flare and of course high uric acid...My nephrologist advised me to get all of kidney related tests done, 24 hour urine protein, rft, creatine to urea ratio etc.....at first there were traces of protein but then it cleared once the gout attack ended.

I have noticed this issue for the past few weeks, I had a scab on my leg from a 3 weeks old injury (a really small tiny one like the size of this 'o', and it started bleeding when I was taking a bath, it kept bleeding the whole time, down to my feet and floor......Then I applied firm pressure on it, an adhesive bandage as well but it still kept bleeding for 3 hours, my white trousers were all covered in blood....3 days ago I got a mosquito bite on my leg, scratched it and it started bleeding abnormally...Funny enough, my hands and fingers never bleed, even for the finger p

Hi,
Got Blood CP: Hb 14, RBC & MCV elevated.
RFT, GFR (110+), uric acid, thyroid (T3/T4 normal, TSH 0.5), autoimmune profile & ESR all normal.
No recent LFT — 3 yrs ago GGT was mildly raised.

Fingers don’t bleed (even for pricks), but legs/feet bleed a lot from minor scratches — should I get INR tested?
Also, for 6 months my cycle has changed: severe anxiety/depression a week before, day 1 spotting, day 2 bleeding, day 3 abrupt stop, then light spotting till day 7 (I used to have full 7-day periods).

Wanted to check if any tests are needed before I book an appointment. Thanks!

There are number of causes of bleeding in renal disease, mainly platelet dysfunction, you need to optimize the control of kidney disease, also keep your uric acid levels within normal range as it can exacerbate kidney damage, you can take Tab allopurinol for this ,avoid any blood thinking medicine and medicine that can exacerbate renal disease also keep your Hb within normal range
Get labs like cbc,Liver functions INR,and renal functions done

For menstrual disorder you need to get LH,FSH,Prolactin levels on day 2-5 of your cycle along with pelvic ultrasound
bleeding pattern is quite unusual it might suggest a vascular cause anyhow you need a basic work up like bleeding time,Aptt INR and Liver function tests, Renal functions ,platelet count
