Asking For Wife, Female 26, Shorkot
My wife feels pain in head because of a migraine. if she takes medicine then her stomach can't bear it. she also has a PCOS. and now she daily feels fever mostly at evening time. she also has joint pain. We were trying to fix this problem for 8 to 10 months.Now We are tired of taking medicine. and these days she is really upset and I feel like she is in a depression.

she needs a full work up if she gas frequent episodes of migraine she can take preventive treatment fir joint oajn a detailed history and investigations accordingly are required

But for now few tips that I can give you are : your wife needs to improve her sleep, start regular exercise, use a healthy diet (proteins, fruits, vegetables), lose weight if overweight, get proper sunlight, avoid stress, try to avoid conflicts and fights and try to be happy, use stress-relieving techniques like deep breathing, positive imagery and meditation etc. She should also see a psychologist or a counsellor. She might also need some tests to look for diseases like thyroid, diabetes, anaemia etc. She might need to be started on migraine prophylaxis, antidepressants, PCOS medicine, vitamins supplements etc

Immature Platelet Fraction (26%)
Thyroid Profile( TSH: 2.1 µIU/mL, Free T4: 14.6 pmol/L)
Fertility / Hormonal Profile (Prolactin: 35.6 ng/mL)
ANA (Negative)
Ultrasound Abdomen & Pelvis (Bilateral Polycystic Ovaries)

Looks like she might have ITP. Her thyroid is fine and HB is not that bad. Avoid routine painkillers like Diclo, Brufen, Synflex, Nims, Ponstan, Disprin bcoz they can reduce the platelets further. Avoid intra-muscular injections.

I can understand you have done some work up but as I mentioned before need to proceed with more specific investigations and then medication to control symptoms and not to affect quality of life
Send me all work up done so far. All medications she is taking . After this I will be in position to advise further. She must be fully evaluated by taking her history . If she is a case of depression having psychosomatic symptoms,she will need opinion of psychiatrist and internist too.
