Asking for Someone Else, Mother, Female, 55 years old, Larkana
Hello, My mother is a patient with diabetes and high blood pressure. Recently, her eyes have become red due to an infection, and her gums have become swollen, causing severe and unbearable pain. I am very worried about her.Her blood pressure is often high, usually around 180/76 mmHg, and yesterday her blood sugar level was 186 mg/dL. She is currently taking Covam 5 mg/160 mg for blood pressure and Gilo-P for diabetes. Could you please tell me whether these medicines are appropriate for her condition? She also has a fatty liver and is now suffering from gum and dental problems. The doctor has prescribed Augmentin, Nise, and Gramerx. Hr face is swollen. I would appreciate your advice regarding her medications and overall condition.

needs evaluation of detailed history of co-morbid conditions, duration of these co-morbid conditions,examination as well as ascertaining compliance to therapy. Treatment has to be tailored as per organ involvement. Her cardiac and renal status needs to be evaluated specifically.

Try to monitor BP and sugar at home if still persistently high consider increasing Covam to 10/160. Regarding diabetes go for HbA1C if still high diabetes medications can be changed . For gums I can not hive much advice but if gums are affected due to infection then controlling daibetes might help.

The patient needs extra care for oral hygiene in addition to the above treatment.
salt water gargling and rinses are advised 3 times a day.
brushing teeth with a soft brush is
required after every meal to remove all food particles.
Remove all bakery items, salted biscuits and white flour dishes from the diet.
Daily walking for 3- 5 minutes 3-5 times a day will improve blood pressure and sugar control.

May she get well soon. Sounds like she might be suffering from a severe viral infection causing gingivostomatitis and conjunctivitis - I’m worried it could be Herpes infection. Or it might be something else like a severe drug reaction. Did she recently start any medicines before this illness? She should get checked by an ophthalmologist, and dermatologist. She might need antivirals or steroids as well. Regarding her BP if it remains uncontrolled you can increase to Covam to 10/160 and if still remains high then later switch to Covam plus 10/160/12.5. Similarly for the Diabetes she might need medicine adjustments. It’s better to consult a Medical specialist for this.
