Talk to Oral and Maxillofacial Surgeon on Oral Submucous Fibrosis

Asking for Someone Else, Brother, Male, 30 years old, Lahore

I am a 28-year-old male recently diagnosed with Oral Submucous Fibrosis (OSMF/OSF). My doctor has noted involvement of the labial mucosa, buccal mucosa, faucial pillars, and soft palate, with mouth opening approximately 30–35 mm. I have had symptoms for around 2–3 years. My left cheek feels noticeably harder and less stretchable than the right, I have some whitish areas/patches inside the mouth, and I cannot comfortably tolerate very hot tea, although my mouth-burning sensation is not severe. I have completely stopped supari/areca-nut use and do not intend to restart it. I am currently being treated by an Oral & Maxillofacial Surgeon with Lycopene 25 mg, Pentoxifylline 400 mg twice daily, and weekly intralesional injections containing local anesthetic/lidocaine, Triamcinolone 40 mg, and Hyaluronidase. Based on these clinical findings, is this diagnosis and treatment plan considered appropriate and evidence-based for my likely stage of OSMF, what stage would I most likely fall into, should the unilateral cheek hardness or whitish patches require biopsy or evaluation by an Oral Medicine/Oral Pathology specialist, should structured mouth-opening physiotherapy be added, and what realistic improvement in mouth opening and disease control can be expected with continued treatment and complete habit cessation?
