Sir, my 2-year-old child is being treated for active nephrotic syndrome. Steroid treatment was started on 26 May. According to the latest reports: UPCR is 17.7, urine protein is nil, serum albumin is 4.6 g/dL, creatinine is 0.33 mg/dL, and RFT is normal. Hemoglobin has improved from 8.7 to 9.3 g/dL. However, TLC/WBC has increased from 18,000 (before steroids) to 21,000 after one month of steroid therapy. ALT is 58 U/L. Could this elevated WBC be due to steroids alone, or should we evaluate for an underlying infection? Do you recommend any further tests or treatment?
