Talk to Psychiatry on Memory, Focus, Concentration

Hello,. I had been facing problem concentrating larger part of my life and I easily get distracted no matter how important the task is. But its been a year that My memory is going down badly to an extend that during conversations i forget very basic words and names even if i have had practiced it before and i stumble in a noticeable way that becomes awkward and embarassing for me. And sometimes i just zone out completely during official meetings/ discussions. Happens same in informal situations as well, i ask questions and zone out while its answered and after a while i ask again. My brain has stopped working out basic mathematical calculations too. I wouldnt call my memory great before that but now it has become very alarming. What could be possible causes and which doctor should i go to or any tests i should get done. Thanks
