I pray that you get well soon. But you need to consult a medical specialist in person. For now it appears you have multiple nutrient deficiencies - for which we need to look for the causes especially gut diseases and you should also get a Nutritionist evaluation; secondly it appears you have multiple chronic pains especially Migraine and Sciatica-like pain - this could be due to the nutrient deficiencies or independent of these - you will need long term prophylactic medicines for pain control and also Physiotherapist consultation; thirdly it appears you have Anxiety disorder - this might also be caused by the nutrient deficiencies, anaemia or it might be independent of these - for this you might need to start anxiolytics and get a Psychologist consultation. You also mentioned raised TSH - this will need to evaluated as well. In the meantime while you await an in-person consultation from a Medical specialist, you should start to implement lifestyle changes like healthy high protein diet, regular mild exercise, good sleep, avoid mobiles/social media use, meditation, sunlight exposure and continue Iron/Vit-D/Calcium supplements for now.

You have mixed up day to day symptoms which everyone can have it. It is better you do baseline blood work as you have not done it then consult Internal medicine specialist plus psychiatrist. You will need some medications ? if there is some issues with blood, endocrine bloodwork up plus psychiatrist opinion with anxiolytic/ antidepressants medications too.
